Though quite possibly unrelated reasons, it would be an unfortunate coincidence if this was caused by a fatigue crack. The previous time it was by a maintenance error in the way they attached the engine.

Though the crack could have been missed in the last inspection but a GVI 4 years ago harbors no evidence anymore if someone was missed or seen nor would anyone remember it.

Yes. We don’t know why the engine tore itself off, but it looks like that started the event, not an uncontained engine fire or something like that.

Yeah that never really made much sense with how fast this developed. Not surprised the FAA grounded the whole fleet either.

Fatigue cracking is quite hard to detect, we used either ultrasonics or dye penetration testing,

The dye pen only works on surface cracking, but the ultrasonic works with internal cracks but you have to have access to the whole part
